Virtually every application will require an official copy of your high school transcript. Your transcript includes:
- Your classes
- Semester grades
- Cumulative weighted grade point average
For it to be official, it must have the school seal and be mailed directly from MHS. Allow enough time for processing (at least two weeks) and the time necessary for your transcript to arrive on time. Requests are date-stamped and handled on a first come, first served basis.
To complete a transcript request form, you will minimally need:
- Institution Name
- Address of Institution
- Parent/Guardian Signature
If the institution does not require a letter of recommendation from a counselor, students must fill out a Transcript Request Form and turn in any counselor forms that need to be completed by their counselor.
